    Hello doctor, I had a sudden fall in washroom and my lower back got injured. I had Mri which showed diffused disc bulging at l4-l5 level and central disc protrusion at l5-s1 level mildly intending the thecal sec.
    I can’t stand or walk for long can’t do bending and heavy work. It’s been one year and it’s not getting improved.

    vertebral bodies are normal in height,intensity and alignment.
    partial disc desiccation are noted in L4/ and L5/S1
    DIffuse posterior disc bulge with left posterocentral to paracentral disc extrusion with caudal migration is seen at L4/5 level indenting the thecal sac and bilateral exiting l5 nerve(grade II on right,grade III on left).bilateral facet joint hypertrophy is seen which along with discpoathy is causing spinal canal narrowing(canal AP DIAMETER 7.6MM) L4/5 posterior interspinous gap edema is seen.
    diffuse disc bulge with small posterocentral disc protrusion is seen at L5/SI Level indenting the thecal sac and with no neural foraminal compromise. bilateral facet joint hypertrophy is seen

    My sister had her back scanned and the diagnosis was as follows:

    “Posterior annular bulge with broad-based posterior central and right para central zones; herniation of l4-l5 inter vertebral disc causing anterior thecal sac indentation. Moderate spinal canal compromise with mild right l5 nerve root impingement. Moderate l4-l5 inter vertebral degeneration is seen. Moderate l4-l5 and l5-s1 facetal orthrosis is noted. Mild spondylotic changes are seen in cervical and lumbar spine.”

    The doctor we consulted thinks that this is not curable and that complete rest is the only option available. Would like your expert opinion. Is this treatable by homeopathy?

    Sir,
    I am 29 year old & suffering with herniated disc problem. presently i am getting pain in lower back and left leg and unable to walk freely as well as stand for more than 5 min. also i am getting pain while i am setting for more than 30 min. i had taken bed rest for three week but still pain is getting severe. i was getting treatment from a neurosurgeon, and he recommended me the surgery.
    my MRI Report reveals the following :

    There is loss of normal lumbar lordosis. The marrow within the vertebra reveals normal signal intensity. No focal bony is detected. L5 vertebra is sacralised. Small marginal osteophytes are detected from L1 to L 5.
    There is mild restrolistesis of L4 vertebra over L5.
    L4-5 disc is degenerated. It reveals posterior and left paracentral herniation and causes compression of the thecal sac and both the L5 nerve roots(left > right).
    L2-3 and L3-4 disc reveals mild deffuse posterior bulge. These efface the thecal sac.
    Small posterior bulge of D12-L1 disc is also seen. It efface the anterior subarachnoid space.
    Facet joint are normal. There is no thickening of ligamentum flaum.
    The lower end of the spinal cord , cauda equina and falum terminale do not reveals any abnormality. No abnormanilty is detected in prevertebra region. The vascular structure appear normal.
    A perineural crst is seen at S1 vertebral level.
    Spinal canal measurement are as follows:
    L1 – 15 mm
    L2 – 15 mm
    L3 – 15 mm
    L4 – 13 mm
    L5 – 13 mm
    There is diffuse posterior bulge of C4-5. D-2-3 to D5-6 disc also reveals mild posterior bulge.
    IMPRESSION :
    • Posterior and left paracentral herniation of L4-5 disc
    • Mild posterior bulge of D12-L1, L2-3 & L3-4
    • Mild restrolistesis of L4 vertebra over L5.
    • Sacralisation of L5 vertebra.

    FINDINGS: MRI of the lumbar spine was performed. There are no previous comparisons available for correlation.

    The current study demonstrates some soft tissue irregularity seen involving the posterior soft tissues and elements at approximately the L5-S1 level. Whether this represents previous surgery or not is not clear. If there has been previous surgery, perhaps a contrast-enhanced MRI can be of help.

    At this L5-S1 level, there is a soft tissue structure indenting the ventral and left side of the thecal sac measuring approximately 5-6 mm with mild left-sided spinal stenosis and significant left lateral recess narrowing. This is probably a disk herniation, although if there has been previous surgery, postsurgical epidural scarring with granulation tissue changes may appear similarly and a contrast-enhanced MRI can help differentiate this.

    At the L3-L4 and L4-L5 levels, mild posterior annular disk bulges slightly indented the ventral surfaces of the thecal sac without significant spinal stenosis. There is mild left lateral recess narrowing seen at the L4-L5 level.

    There is mild facet arthrosis seen narrowing the neural foramina, especially on the left at the L5-S1 level.
