3. Pain: Varicocele is usually painless but in a few cases, might also cause pain. The pain in the scrotum is usually dull, throbbing-like but rarely sharp in nature. Increased temperature in testicles, increased pressure in scrotal veins and stretching of nerve fibers in the spermatic cord from dilated veins may be a contributing factor in causing pain. The pain may worsen as the day progresses, while standing or after physical exertion. Pain may get better by lying on the back. The testicles may feel heavy as well.

4. Size: The affected testicle may look smaller than the other.

What are the causes behind Varicocele?

1. There are two testicular arteries that supply oxygenated blood to testicles and two testicular veins which carry deoxygenated blood from testicles to heart. There is a network of small veins called pampiniform plexus in the scrotum (which holds the testes) that carries deoxygenated blood from testicle to testicular vein. The reason behind Varicocele is not clear yet. However, it is thought to arise from pooling of the blood in the veins of pampiniform plexus. This happens when the valves inside the veins of spermatic cord, that allows one-way blood flow, do not work the right way as they should. It prevents blood from flowing properly in needed direction resulting in the back (reverse) flow of blood causing the veins with in scrotum to widen and enlarge resulting in Varicocele.

Spermatic cord is a tube that hangs testes in scrotum and contains testicular artery, lymphatic vessels, pampiniform plexus, nerves and vas deferens which carry sperms from testes to penis).

2. Deep vein thrombosis (a blood clot in one of the deep veins of body usually in legs), thrombosis (blood clot within blood vessel) of pampiniform plexus are some of the rare causes of Varicocele.

3. It usually affects males between 15-25 age group.

4. Tobacco smoking may increase risk of Varicocele.

Classification

Based on size and clinical examination these are classified as follows:

1. Large varicoceles: These can be easily detected by simple inspection (looking over) and appear like bag of worms.

2. Medium varicoceles: These are the ones that are detected by palpation with fingers even without any bearing down by patient.

3. Small varicoceles: These varicoceles can only be detected during strong bearing down (valsalva).

Another classification is as follows:

1. Pressure type Varicocele: In this spermatic vein fills with blood resulting in grade I Varicocele.

2. Shunt type Varicocele: In this intense build-up of blood damages spermatic vein and also other veins causing larger varicocele including grade II and III.

Grading system of size of Varicocele

Grade 0 – When varicocele is not detected physically and can only be seen on conducting an ultrasound of scrotum

Grade I – When varicocele is felt only on palpation (checking with fingers) during valsalva maneuver when standing

Grade II – when varicocele is palpable when standing even without valsalva maneuver but not visible

Grade III – When it cause visible scrotal deformity i.e. varicocele is visible through scrotal skin

Grading as per colour Doppler, given by Sarteschi, is as follows:

Grade I – In this reflux occur at groin level during valsalva. There is no testicular enlargement.  Deformity of scrotum is absent.

Grade II – Reflux occurs at the level of proximal part of pampiniform plexus only during valsalva. There is no testicular enlargement or deformity of scrotum.

Grade III – In this grade, reflux occurs in distal vessels at level of lower scrotum during valsalva. There is absence of testicular enlargement or scrotal deformity.

Grade IV – There occurs spontaneous reverse blood flow increasing during valsalva and in addition to this, scrotal deformity and testicular hypertrophy (enlargement) may also be there.

Grade V – There occurs reflux in dilated pampiniform plexus at rest that increases during valsalva and is always attended with testicular enlargement.

Complications

1. Impaired sperm production: A certain body temperature is required in testes for normal good quality sperm production, maturation and function. The heat in scrotum is approx. 5degree less than that of the belly. The pampiniform plexus works to cool down blood in testicular artery prior its entry to testicle. If there occur enlargement of veins in pampiniform plexus its normal functionality drops resulting in overheating in testes that affects normal production of sperms. When microscopic semen examination is done in case of Varicocele, stress pattern is seen. The sperm count becomes low, the sperms show poor motility and there is increase in number of abnormal sperms.

2. 4 out of 10 males having Varicocele face infertility.

What other conditions needs to be ruled out when making diagnosis of varicocele (differential diagnosis)?

1. Hydrocele: Swollen scrotum from collection of excessive fluid in the sheath (tunica vaginalis) surrounding a testicle.

2. Epididymitis: Inflamed epididymis (a coiled tube at the back of testicle that stores sperms while they mature and carries sperms from the testes to the vas deferens).

3. Spermatocele: cyst developing inside the scrotum

4. Trauma

5. Testicular and paratesticular tumour

6. Testicular torsion: An emergency condition in which spermatic cord that carries blood to testes rotates and twists

7. Inguinal hernia: Bulging of the contents of the abdomen through a weak spot in lower abdominal wall

Diagnosis and Investigations

1. Physical examination: A physician can make a clinical diagnosis by visually checking scrotum and by palpation (checking with fingers and hands of scrotum). In many cases, Varicocele cannot be seen or felt in lying down position. So the doctor may also examine scrotum in standing position.

He may ask you to stand up, take a deep breath in standing position and to hold the breath and bear down to do the scrotal examination and check for any enlarged veins in it. It is called as Valsalva maneuver.

2. Scrotal Doppler ultrasound: Doctor may recommend doppler ultrasound of scrotum to confirm diagnosis and rule out any other cause of the symptoms.

Veins of pampiniform plexus wider than 3mm with blood flow in the wrong direction during valsalva maneuver is indicative of Varicocele. If it is Varicocele, grading will also be done with its help. Ultrasound also checks the size of testicles.

3. In case of Varicocele on the right side, it is important to rule out any mass or tumour in abdomen that may be the cause. There is a possibility of renal cell carcinoma in case of isolated right-sided Varicocele. It could be occurring from vena cava obstruction from right renal cancer vein tumour thrombus. If suspected CT scan is recommended.
