2. Side: Varicocele mostly is one-sided. Either side can be involved. However, chances of left-sided Varicocele remain high. Varicocele is 80-90 per cent times common in left testicle. In rare cases, both sides can be involved.

3. Pain: Varicocele is mostly painless but in some cases, it might cause pain. The pain in the scrotum is usually dull, throbbing but rarely sharp. Increased temperature in testicles, increased pressure in scrotal veins and stretching of nerve fibres in the spermatic cord from dilated veins may be a contributing factor that results in pain. The pain may aggravate as the day progresses, or if one is stationary or after physical exertion. There may be relief from pain by lying on ones back. The testicles could also feel heavy.

What are the causes behind Varicocele?

1. There are two testicular arteries that supply oxygenated blood to testicles and two testicular veins that carry deoxygenated blood from testicles to heart. There is a network of small veins called pampiniform plexus in the scrotum (which holds the testes) that carries deoxygenated blood from testicles to testicular vein. The reason that leads to Varicocele is not clear yet. However, it arises from pooling of the blood in the veins of pampiniform plexus. This happens when the valves inside the veins of spermatic cord, that allows one-way blood circulation, do not work the right way. It prevents proper blood circulation in the proper direction that it should resulting in the back (reverse) flow of blood causing the veins within the scrotum to widen and enlarge resulting in Varicocele.

2. Spermatic cord is a tube that hangs testes in scrotum and contains testicular artery, lymphatic vessels, pampiniform plexus, nerves and vas deferens that carry sperms from testes to penis.

3. Deep vein thrombosis (a blood clot in one of the deep veins of the body usually in legs), thrombosis (blood clot within blood vessel) of pampiniform plexus are some of the rare causes of Varicocele.

4. It usually affects men between 15-25 age groups.

5. Tobacco may increase the risk of Varicocele.

How is Varicocele classified?

–Based on size and clinical examination these are classified as follows:

1. Large Varicoceles: These can be easily detected by simple inspection (looking over) and appear like bag of worms.

2. Medium Varicoceles: These are the ones that are detected by palpation (using fingers to clinically observe the patients).

3. Small Varicoceles: These Varicoceles can be detected during strong bearing down (valsalva).

— Another classification is as follows:

1. Pressure type Varicocele: In this spermatic vein fills with blood resulting in grade I Varicocele.

2. Shunt type Varicocele: In this, intense build-up of blood damages spermatic vein and other veins causing larger Varicocele including grade II and III.

 Grading system of size of Varicocele

Grade 0 – When Varicocele is not detected physically and is only visible on conducting an ultrasound of the scrotum

Grade I – When Varicocele is felt only on palpation (checking with fingers) during valsalva maneuver when standing

Grade II – when Varicocele is palpable when standing even without valsalva maneuver but not visible

Grade III – When it cause visible scrotal deformity i.e. Varicocele is visible through scrotal skin

Grading as per colour Doppler, given by Sarteschi, is as follows:

Grade I – Reflux occurs at the groin level during valsalva. There is no testicular enlargement, without any deformity of scrotum.

Grade II – Reflux occurs at the level of the proximal part of pampiniform plexus only during valsalva. There is no testicular enlargement or deformity of scrotum.

Grade III – In this grade, reflux occurs in distal vessels at the level of lower scrotum during Valsalva, without testicular enlargement or scrotal deformity.

Grade IV – There is spontaneous reverse blood flow which increases during Valsalva. Scrotal deformity and testicular hypertrophy (enlargement) may be seen.

Grade V – The reflux in dilated pampiniform plexus occurs at rest that increases during valsalva and is always attended with testicular enlargement.

What are the complications?

1. Impaired sperm production: A certain body temperature is needed in the testes for normal good quality sperm production, maturation and function. The heat in scrotum is approx. 5 degrees less than that of the belly. The pampiniform plexus works to cool down blood in the testicular artery prior to its entry to testicle. If there is enlargement of veins in pampiniform plexus its normal functionality drops resulting in overheating in the testes which affects normal production of sperms. When microscopic semen examination is carried out in the case of Varicocele, a stress pattern is observed. The sperm count becomes low, the sperms show poor motility and there is an increase in the number of abnormal sperms.

6. Testicular torsion: An emergency condition in which spermatic cord that carries blood to testes rotates and twists.

7. Inguinal hernia: Bulging of the contents of the abdomen through a weak spot in the lower abdominal wall.

How to Diagnose Varicocele?

1. Physical examination: A physician can carry out a clinical diagnosis by visually checking scrotum and by palpation (examining scrotum by using fingers and hands). In many cases, Varicocele cannot be seen or felt in a lying down position. So the doctor may also examine the scrotum in a standing position.

2. He may ask you to stand up, take a deep breath in standing position and to hold the breath and bear down to do the scrotal examination and check for any enlarged veins in it. It is called the Valsalva maneuver.

3. Scrotal Doppler ultrasound: Doctors may ask for a doppler ultrasound of scrotum to confirm diagnosis and rule out any other cause of the symptoms.

4. Veins of pampiniform plexus wider than 3mm with blood flow in the wrong direction during valsalva maneuver are indicative of Varicocele. If it is Varicocele, grading will also be done with its help. Ultrasound also examines the size of testicles.

5. In the case of Varicocele on the right side, it is pertinent to rule out any mass or tumor in the abdomen that may be the cause. There is a possibility of renal cell carcinoma in case of isolated right-sided Varicocele. It could occur from vena cava obstruction from right renal cancer vein tumor thrombus. If suspected, CT scan is recommended.

    Some time i feel small pain at my left side testees not regular. My marriage date is 14.02.2016 i try from that time but not get any result for baby

    My total sperm count before treatment on 14.09.2016 is 15 million/ml

    Hello Dr. I am 44 years old & I am suffered from vericeles both side last 20 years. For which i had to surgery left side in 1998 after that i seen the right is also having samelike pain, little swelling,sensation etc problems. A lot of pain and burning, sensation on right side for last ten years, also having itching abnormality in the anus. Please prescribe me the best treatment & medicines.

      M last 3 yr se varicocele se grashit hu 2014 k last m surgery bhi ho chuki h lekin varicocele problem ab bhi face kr rha hu last 2 yr se bakson hospital ki homeopathic medicine bhi chl rhi h lekin koi relef nhi h… Mujhe phle heavy wait feel hota tha kuch homeopathic medicine k bad wo kafi hd tk thik h abhi testic m pain aur right side wale testic m jyada prblm face kr rha hu…. Varicocele left side se shuru hua tha abhi right side m jyada prblm face kr rha hu blood circulation bhi thik nhi h plz sugges me.. my age that time 22 varicocele start 2013.

    I am satyendra yadav from India panja. Mai 2salo se vericocile she grasit hu bahut jagah dikhaya bhi fir bhi kuchh thick nahi h.English medicine bhi khaya phir bhi think nahi huaa.vericocile normal h.left testis chhota h aaur right wala bada h.give any advice please what I do.

  19. Hi sir I have a varicose veins problems in my both testes from the last one year. I am in discomfort. My testes are hanging and feeling severe pain in my both testes. I am also feeling pain in my abdomen and gall bladder. So I want to ask you, if I start using homeopathic medicine it will cure my problem? How much time it will take? Is it expensive?     I was dignosed with varicocele 2 month ago, when I felt pain in the testicles and problem in passing urine. The varicocele was confirmed by Doppler ultrasound. The allopathic doctor decided not to do surgery now as I feel little pain while sitting or standing for long hours. He also told me if pain in the testicle increases only then we will do the surgery. I still feel the heaviness and mild pain in the scrotum. I also feel pain during ejacuation. Kind suggest me homeopathic treatment so that this problem will be completely cured without surgery.
