It is estimated that PANDAS accounts for approximately 1-200 cases of OCD in children every year. Some researchers suggest categorizing PANDAS under PANS (pediatric acute-onset neuropsychiatric symptoms) or CANS (childhood acute neuropsychiatric symptoms). PANDAS is a pediatric disorder, and the children between the ages of 3 years to puberty are chiefly affected by this disorder. It is very unlikely that an adult would suffer neuropsychiatric symptoms following a streptococcal infection.

Cause of Paediatric Autoimmune Neuropsychiatric Disorder

Various studies show that group A streptococcal bacterial infection causes an immune response that results in different neuropsychiatric symptoms associated with PANDAS. Group A streptococcal bacteria is the same bacteria that causes strep throat. It is considered that strep bacteria tend to hide from the body’s immune system and survive in the human host for a long duration. It does so by putting molecules on its cell wall that mimics the molecules found on a child’s skin, brain tissues, joints, and heart. Due to the similarity of the cell membrane of bacteria and the locations mentioned above, the immune system is not able to detect the bacteria for a long time. When our immune cells recognize these foreign bacteria, they start producing antibodies to fight against them. As the molecules on bacteria cell wall mimic human host molecules, the antibodies also start attacking the body’s healthy tissues along with the bacteria. In some cases, the brain cells are attacked, resulting in neuropsychiatric symptoms appearing in children having PANDAS.

Symptoms of Paediatric Autoimmune Neuropsychiatric Disorder

There are many symptoms that are noted in cases of PANDAS. The major symptoms of PANDAS include OCD (obsessive-compulsive disorder) and tics. The OCD symptoms include frequent hand washing, ritualistic behavior and many more.
Tics refer to semi-voluntary repetitive movements or sound production that involves distinct muscle groups. The tics like throat clearing, eye twitching, and head shrugging are frequently seen in PANDAS. In addition to these ADHD symptoms may arise. Other attending symptoms include anxiety, refusal to go to school, separation anxiety (child wants to cling to the parents or caregiver). Mood changes may appear, like sadness, irritability, unexpected crying or laughing, using harsh words, and screaming for hours together. Childish behavior, inability to focus, lack of attention, and memory weakness are also noted. Other symptoms include sensitivity to clothes, sound, light, loss of appetite, a decline in fine motor skills and sleep issues. A decline in academics, developmental regression (child behaving much younger than his/her actual age), poor mathematics and bad handwriting are some other issues that may arise in a child having PANDAS. Frequent urination and bedwetting may also be present.

Diagnosing PANDAS

There are no laboratory tests available that can diagnose a given case of PANDAS. PANDAS is suspected in a child based on the sudden onset of neuropsychiatric symptoms following a streptococcal infection. Following are the guidelines that help in diagnosing PANDAS:
1. Presence of OCD (obsessive-compulsive disorder) and/or multiple tics.
2. Very sudden and abrupt onset/ worsening of symptoms.
3. The onset of the symptoms between age 3yrs to pubertal age group.
4. Association with group A streptococcal infection.
5. Neurological abnormalities (physical hyperactivity or unusual jerky movements).

PANDAS and Autism

PANDAS and Autism are two different disorders. However, these two conditions can coexist in some children. But there is no clear evidence regarding the percentage of occurrence of PANDAS in children with autism as compared to those who do not have autism. It is also difficult to reach a diagnosis for PANDAS in a child with autism. This is because of the symptom similarity that these common disorders share with each other. The major symptoms shared include repetitive behaviors, sensitivities (towards touch, light, sound). One differentiating point to be noticed here is that the symptoms in PANDAS develop abruptly and all of sudden as compared to autism.

    My 16 year old daughter was previously diagnosed with PANDAS and is experiencing prolonged anxiety coupled with recent inability to eat, leave the house, and muscle twitching. I am very concerned because every doctor just wants to medicate her and nothing is working. She has been on Effexor for several months and is now resorting to vaping THC to calm herself.     My son has Cunningham Panel indicating Pandas. He is 22 with medium autism and huge OCD. Everything boils down to such fear of failure, he can’t hardly move. Catatonia. Movement disorder – keeps circling back and can’t reach destination. We had five wonderful days using a homeopathist’s “virus” with “BrainDrops” by Professional Formulas company. Then the magic ended. Generally better – no extreme distress, but the catatonia is still here. I don’t know what I’m doing – I better ask somebody because this could work!!!!
