Types Of Urinary Tract Infection

An infection can happen in any part of the urinary tract, following are the types of UTI:

Urethritis: An infection in the urethra is known as urethritis.

Cystitis: An infection in the urinary bladder is referred as cystitis.

 Pyelonephritis – Infection in the kidneys is referred as pyelonephritis.

Pathophysiology And Causes Of UTI

A urinary tract infection arises when bacteria gain access to the urinary tract and begin to multiply there. Escherichia coli, commonly known as E.coli, (a type of bacteria commonly found in Gastrointestinal tract, GI) is the most common bacteria infecting the urinary tract. The bacteria mainly enter the urinary tract via the urethra and gut. Other less commonly known bacteria causing UTI are Staphylococcus, Pseudomonas and Klebsiella.

Cystitis is mainly caused by Escherichia coli (E coli), 90 % of bladder infections are caused by E coli, however, sometimes other bacteria are also responsible.

Urethritis: This type of UTI occurs when GI bacteria spread from the anus to the urethra. Also because urethra in females is close to vagina, sexually transmitted infections such as gonorrhoea, herpes, Chlamydia and mycoplasma can cause urethritis.

Risk  Factors

Following factors make a person more prone to UTI. Women are more likely to contract this disease, risk factors specific to women for UTI are mentioned here:

1. Anatomy of female: The length of urethra is short and its orifice being quite close to the anus in women, makes it easier for the bacteria to spread to the bladder. This leads to higher chances of acquiring a urinary tract infection in women as compared to men.

2.Menopause: After menopause, there is a decline in circulating oestrogen that causes changes in urinary tract that makes the females more prone to UTI. Some women have urinary tracts that makes the bacteria easy to cling to them and are genetically prone to UTIs.

3.Sexual activity: Sexually active women suffer from UTI more times than women who are not sexually active.

4. Birth control agents:  Women using diaphragms for birth control and also those using spermicidal agents are at higher risk of UTI.

Other risk factors include:

Suppressed  immune system:  People who suffer from conditions like diabetes (high blood sugar) are more likely to develop UTIs because their bodies are less capable of fighting infection effectively.

Obstruction in the urinary tract:  Prostatitis, or obstruction of the urethra by an enlarged prostate, can cause insufficient bladder emptying in men which raises the risk of infection. Most elderly men experience it.

Usage of Catheter– People who are unable to urinate on their own and use a tube(catheter) to pass urine are at more risk of developing UTI.

Symptoms Of UTI

Symptoms of lower Urinary tract infection (bladder and urethra):

1. The most typical symptom is burning or pain when urinating.

2. Increased urinary frequency without passing much urine.

3. Strong need or urge to urinate but only producing a few drops of urine.

4. Urine has a foul odour and is cloudy.

5. Non-visible or visible haematuria (urine can appear bloody)

6. Pain in the lower back, lower abdomen, and even the pelvic region will typically make you feel the need to urinate more frequently.

Upper UTI Symptoms: It affects kidneys, the symptoms are:

1. Fever and chills

2. Nausea and Vomiting

3. Flank Pain (pain on either side of lower back between pelvis and ribs)

What tests would have to be conducted to confirm UTI?

    The main problem with you is that you never reply to urgent questions. My problem is that I am suffering from chronic bacterial prostatitis for more than 30 years. Now my age is 67+. From 1998 I am consulting many MD, MCH, MS, FRCS urologists and even had undergone TURP in January 2001. Everytime urine culture confirmed high colony of E. coli. Doctors tried short to prolonged course of all possible antibiotics. But as soon as the antibiotic was stopped, infection returned. From 2009 onwards climbed upto both kidneys and created two cysts in two kidneys. From that time onward, nature of fever has also changed – 105 to 106 F. Bacteria came to blood stream causing septesemia and infected other organs causing stubborn constipation, impaired liver function, heart problem, insomnia, severe fatigue and loss of memory. The situation became life threaning in January 2019. Tazar 4.5 IV drip and high dose of oral Levofloxacin 750 mg were administered to save life. However, when antibiotics were stopped after 2 and half months, the infection returned with full virulence.

    Now I am taking Colibacillinum 200c 5 drops twice daily along with R87 15 drops twice daily. It seems to have given me some relief. I want to stop R87 after one month and continue with colibacillinum 200c for 6-8 months, after which I want to shift over to colibacillinum 1M for a long period to completely get rid of E. coli infection.
    Now my constipation has gone. I have regained some strength and also anxiety and depression has gone down. Now I am having good sleep for 7 to 8 hours, but the body clock has changed. Sleep does not come before 2/3 AM and continues upto 8/9 AM. Liver function seems to have improved, though I have not tested SGPT / SGOT values recently. BP has also come down to around 150/80 from previous 192/85. However, I am taking two BP lowering drugs and rozuvastatin cholesterol lowering drugs.

    My question to is how long should I continue colibacillinum 200c and then colibacillinum 1M? Should I continue for life?

    Dear Doctor,
    I am suffering from chronic bacterial prostatitis for more than 30 years. I consulted best urologists but allopathic antibiotics could not solve the problem. Infection E coli. Last January my condition became very serious and was admitted in hospital with very high fever(105-106 F) and uncontrolled involuntary urination with unbearable putrid smell. This is not prostate cancer as confirmed by cystoscopy, urine cytology and PSA (0.7). I was under strong antibiotics treatment continuosly for two and half months (21 Tazar 4.5gm IV injection for 7 days followed by one Levofloxacin 750 mg tablet daily for two months. The E coli bacteria has infected the blood stream attacking various organs including brain (Septesemia). But the day doctor stopped antibiotic, next day infection returned. All the urologists (of MD, MCH, MS, FRCS stature are confused.

    Am suffering from chronic urethritis and vaginitis from last 2yrs after my daughter was born..
    it’s like first burning urine comes then the itching starts… there is thick stringy discharge which is often transparent with orangish tinge … discharge is also acrid … no odour…it seems it is yeast ….cause the discharge keeps changing to whitish transparent colour..
    first it starts with urethritis then vaginitis happens. Cause urethral discharge slips in vagina and causes the infection ..it’s become chronic now …
