Signs And Symptoms

Initially there is difficulty in writing intermittently but as condition progresses, cramping is felt soon after one starts writing. The following signs and symptoms are noted in Writer’s Cramp:

Tight gripping of pen/pencil

Flexion (bending) of wrist and elevation of elbow

Extension of fingers at times resulting in falling of pen from hand when writing

Discomfort in wrist, fingers and forearm

Lack of muscle coordination of hand, inability to obey commands by hand and fingers

In some of hand dystonia cases, sustained hand posture may occur like clenched fist and bent wrist

Due to above complaints writing becomes untidy

Types Of Writer’s Cramp

1. Simple Writer’s Cramp:  When symptoms appear only while writing and only the ability to write is affected. The hand twists in strange postures while writing.

2. Dystonic Writer’s Cramp: In this type, the symptoms appear even while doing tasks with hands other than writing, for example, it may appear while doing other activities like eating, applying makeup, buttoning a shirt,etc.

Cause

1. It usually starts between age of 30 to 50 yrs. Simple Writer’s Cramp is more common type. It is linked with overuse of hands like excessive writing for years together.  Other factors that play a role are holding pen inappropriately and keeping poor posture of hands while writing. Stress and anxiety can worsen the symptoms.

2. It has been found in research that in persons having writer’s cramp there is decreased brain tissue (gray matter) in those brain areas (cerebellum, thalamus and sensorimotor cortex) which control hand movement on affected side. Though it is still not understood well if these changes are the cause or the result of this condition. Besides, there is a theory that suggest that brain changes might be a part of adaptation due to sustained repetitive hand movement.

3. Dystonic writer’s cramp is less common type and occurs usually as a part of generalised dystonia affecting multiple body parts.

4. Writer’s Cramp may have a genetic component. It is estimated that in about 20% of the persons having writer’s cramp, there is a family member having complaint of dystonia. A gene DYT1 is linked with generalised dystonia.

5. In some cases, hand dystonia may be acquired from some injury, lesions of spinal cord and peripheral nerve damage. In such cases, symptoms are experienced even when hand is at rest, pain may be felt and symptoms may occur in body parts other than hand.

Complications

It includes trembling of hands during cramping and involvement of the other hand in dystonia as well. Another complication that can arise is dystonia in some other body part, like eyelids. In about 50% people having simple writer’s cramp, chances of developing dystonic writer’s cramp are more.

Diagnosis

There are no specific laboratory investigations for diagnosis of Writer’s Cramp. It is diagnosed clinically based on symptom presentation. In some cases. nerve conduction test and electromyography may be recommended if doctor suspects some other medical condition affecting hands and fingers apart from Writer’s Cramp, like Carpal Tunnel Syndrome.

    Since a past few years, I am unable to write as well as do signature. I am now 50 years old and while writing, pen gets slip from my fingers particularly from thumb and I can not hold a grip on it. While writing I have to hold a pen very tight and after certain time it started paining to my thumb. Movements of my right hand and fingers are normal. I have taken Homeopathy medicines from two different doctors but of no use. They suggested me to change position of holding a pen but of no use too.       I have been diagnosed with writer’s cramp in 2015. I am unable to write even single sentence as my wrist bend to right direction due to non movement of wrist in a rythemic way with the progression of sentence in right direction. I also feel extra tight grip with pen
      Since 2018 I have tried with my left hand but there is also some kind of difficulty.
      I have also noted that there is not any difficulty with other daily routine work. In EMG NCV test, strenght of muscle as well as nerve are good but there is some calcium deficiency.
      Please suggest medicine as I am a student and need to treat this problem.

    Sir, I am facing difficulty in writing from April this year. As earlier I am taking medicine for vitamins (as doctor recommended me) 1week ago I gone to neuro doctor so she told me ,you had writters cramps in you hand(right). So she told me their is no treatment for this disease and at present I am student (23yr) so she suggested me to take botox injections for temporary treatment (cures cramps for 5-6month). As I am facing little bit difficulty in gripping pen.their is no pain due to cramp(only some pain occured due to holding pen in different way) . No jerk while writing and also no sweat problem during writing.please suggest me which medicine I can take . Or should I go to local homeopath near me ?

      I have a similar handwritng problem, but carry a heavy set of other far more serious problems. I am 76 years old, have Parkinson’s disease diagonised in 2008, but I knew I had it much earlier as both parents were diagonised so in the early 1980s as was a sister diagonised in 2000. So I learnt from them what to expect. My PD progressed rapidly from 2017, now taking 19 Rytary capsules per day ..Had Deep Brain Stimulation surgery in Aug-Oct 2020. Alas it been very unsuccesful, has made my Parkinson’s Disease much, much worse..

    Sir, my wife is suffering with Writer’s cramp for the past ten years. Fingers don’t have control while eating or while doing any other work. Even tying the hair, wearing saree is a difficult job. While writing the pointing finger is lifted upwards (may be 60 degrees) without her doing so. Can you please suggest some remedy. Normally the pain is not felt, unless worked for a long time. Only inability to perform the work is the issue.
